Routine Care
Hospice staff will provide regular, scheduled visits to a patient's place of residence to adequately manage symptoms and meet the needs of the patient and family.
Continuous Care
Short-term crisis management is provided in the patient's place of residence. Most often initiated when death is imminent and the primary care giver is unable to cope. Hospice staff will provide 24-hour care at this level.
Respite Care
Intended to provide short-term relief for the primary caregiver. The patient can stay at one of our hospice houses for a period of up to five days. Room and board are covered.
In-Patient Care
Short-term crisis management when symptoms cannot be managed in the patient's own home. Patients have an option to have their needs met in one of our community hospice houses. Room and board are covered at this level and all hospice facilities are Medicare certified.

In addition to providing for your care needs, our caregivers offer general companionship and can help facilitate transportation to appointments and family gatherings. A caregiver can help a client maintain hobbies and activities that they enjoy. Private duty care also offers families the necessary peace of mind that their loved one has the attention they need when they cannot be there themselves.
